Chain of Custody (COC)

“Chain of Custody” (COC) is the chronological documentation that records the sequence of custody, control, transfer, analysis, and disposal of a sample. The Chain of Custody document should accompany any samples submitted for testing.

The Master Testing Services Agreement is a document that outlines the responsibilities for maintaining confidentiality and the consequences for breaching those responsibilities. The customer must sign a copy and either email it, submit it to our sample technicians, or print it and mail it to the lab.
